1. Go to /wp-content/themes/themename and open comments.php .
2. Find <?php comment_text() ?> or comment_text() in comments.php and add the following code before that.
That’s it. No need for plugins.

gravatar stands for globally recognised avatar. an avatar is a picture your show in your places like blogger profile etc. If you get gravatar, whereever you leave comments in the web your picture will be shown. for example, you can see the dew drop picture near this comment. that is my gravatr. you can get your gravatr at http://site.gravatar.com/signup
